What should I expect from the Notary Public process?

Senior Members and Cadets 18+

  • Do not sign the form until instructed by the Notary Public
  • Bring an unexpired, government-issued, photo identification; prime examples:
    • State-issued driver’s license
    • State-issued identification card
    • Passport
  • Read the loyalty oath or affirmation at the bottom of the form aloud in the presence of the Notary Public

Younger (<18) Cadets

  • Do not sign the form until instructed by the Notary Public
  • Parent or legal-guardian must be present
    • Must be willing to sign the form (when instructed) on the cadet’s behalf
    • Must bring an unexpired, government-issued, photo identification (see above)
  • Cadet must provide one of the following
    • An unexpired, government-issued, photo identification (see above)
    • An unrelated adult who personally knows the cadet and can sign as a credible witness
  • Cadet must answer the following questions
    • What kind of document are you signing?
      (example answer: this is a volunteer registration form to participate as a disaster service worker in California)
    • What will the document do?
      (example answer: it will allow me to respond as a California disaster service worker to disasters in California managed by CalOES)
    • Do you want to sign the document?
  • Cadet will read the loyalty oath or affirmation at the bottom of the form aloud in the presence of the Notary Public
